ATTENTION ALL ECMS ATHLETES and Parents/Guardians:
You must be on a computer where you can print out these documents!
Here is the Link for the ONLINE forms needed to play Sports at ECMS! Please print (Either on both sides or two pages and staple) and read the information and sign the student's signature and the parent/guardian's signature. The Physical Form must be completed by a Doctor (Setting up an appointment and getting a physical with your local physician before the first practice (first day of School).
